Provider compliance training
Liberty Dental Plan (“Liberty”) monitors and ensures that Liberty’s contracted offices and their staff operate in compliance with applicable laws and regulations, and their contract requirements with Liberty.
Step 1: Complete required trainings
All providers and personnel working with Liberty must complete the required or equivalent trainings within 30 days of hiring or contracting and annually thereafter (per CMS, state, or contractual requirements).
Step 2: Submit completed training attestation
You can submit through our online form or by email, fax, or postal mail.
